Elm & Oak supports low-income individuals, working poor, and other populations served by government-sponsored programs to improve their health status and that of their families. We dedicate our efforts to:

Our Care Management teams include RNs, behavioral health specialists, and community outreach staff, all supported by state-of-the-art systems with customized reporting and analytics. We provide:

Transition care and discharge planning support

Virtual, telephonic, and in-person care coordination

Psychosocial support to address social determinants of health (food, housing, transportation)

Education for clients and their support systems

Culturally responsive engagement tailored to complex medical and behavioral health needs

But our work extends beyond Care Management.

At Elm & Oak, Population Health work is our purpose. It is a coordinated approach to improving outcomes for whole communities by using data, care coordination, and community-based support to identify risks early and address the clinical and social factors that shape health. We combine advanced analytics with hands-on, person-centered support to help partners deliver equitable, proactive care that strengthens both individual well-being and system performance.

And through our Independent Practice Association (IPA), we support over 12,000 providers with tools to succeed in value-based care, offering resources, quality and coding support, and a framework that improves sustainability while keeping care patient-centered.

We also operate one of New York’s most successful Health Home Care Management programs, a coordinated care model that links Medicaid or dual eligible members with chronic or complex conditions to a trusted network of clinical and community partners. This model ensures continuity and trust, closing gaps that traditional systems often miss.

To address low HPV vaccination compliance among young, at-risk members, Elm & Oak partnered with GoMo Health to implement a personalized SMS engagement platform. This initiative led to increased adherence rates across all patient categories, showcasing the effectiveness of tailored digital outreach in preventive care.

QUIC Dashboard: Integrating Behavioral and Physical Health

In collaboration with the Adirondacks ACO, Elm & Oak developed the Quality Utilization and Integrated Care (QUIC) dashboard.
