Our Community Programs provide person-centered care coordination for people facing housing insecurity, behavioral health challenges, or other complex life situations. By pairing trusted partnerships with local outreach, we bring stability and support to those who need it most.

Street outreach that starts with trust.
The Safe Options Support program reaches individuals experiencing homelessness, especially those living with behavioral health conditions, through trauma-informed, voluntary engagement. Licensed clinicians, case managers, and peers work side by side to connect people with housing, care, and community-based resources.

Housing and Behavioral Health
