The Safe Options Support program is a street outreach initiative designed to engage individuals experiencing homelessness, particularly those with behavioral health conditions, through trauma-informed, person-centered support.

Elm & Oak delivers the Safe Options Support Program in the Southern Tier (Steuben/Chemung Counties) and Central New York (Onondaga/Oswego Counties) regions.

Contact Our SOS Team

Our SOS team includes licensed clinicians, case managers, and peers with lived experience. They work together to:

  • Build trust with individuals who are disconnected from care
  • Connect people to shelter, housing, medical and behavioral healthcare
  • Provide ongoing, voluntary case management
  • Advocate for services that promote dignity, autonomy, and safety

SOS operates in collaboration with the New York State Office of Mental Health and other community-based partners to ensure continuity of care and long-term support.
