Connecting Medicaid members to the resources that shape health and stability.
Elm & Oak partners with regional Social Care Networks across New York to help Medicaid members access the supports that make health possible outside the clinic. Through coordinated navigation, we connect people to housing, food, employment, transportation, and other essential resources—and make sure these services are integrated with the care they already receive.
What Social Care Networks Do
Social Care Networks (SCNs) are coalitions of community organizations working together to address health-related social needs. They help identify unmet needs, streamline referrals, and ensure members can access reliable, high-quality social supports.
Elm & Oak works within multiple SCNs to strengthen these connections, remove barriers, and make navigation easier for both members and care teams.
