What Social Care Networks Do
Social Care Networks (SCNs) are coalitions of community organizations working together to address health-related social needs. They help identify unmet needs, streamline referrals, and ensure members can access reliable, high-quality social supports.
Elm & Oak works within multiple SCNs to strengthen these connections, remove barriers, and make navigation easier for both members and care teams.
