This change brings our full body of work under one clear name, reflecting who we are today and how we support healthcare organizations facing increasing complexity. Our mission remains the same: helping our partners deliver better care to the people and communities they serve.
We’re also thrilled to announce the launch of our new website, elmandoakhealth.com.
What’s staying the same
While our name has changed, our mission remains steadfast. You’ll continue to work with the same teams, the same expertise, and the same commitment to practical, human-centered solutions across population health, care management, and provider support. Our legal entities remain in place for contracting, and there is no disruption to services or relationships.

Elm & Oak Health represents a more focused and unified way of sharing the expertise we’ve honed over more than half a century. It sharpens how we support our partners, deepens how we apply our experience, and prepares us to meet the challenges ahead with clarity, consistency, and purpose.
