Read MoreElm & Oak’s IPA connects healthcare providers with a trusted network, equipping them with tools, insights, and support to serve New York’s most vulnerable populations efficiently, compassionately, and with impact.
Whether you’re caring for communities on the front lines or navigating complex health systems, you deserve a partner who’s been there, who gets it, and who’s ready to help.
At Elm & Oak, our mission is to transform healthcare challenges into opportunities for better community health. We blend over 55 years of experience in population health with a personal, on-the-ground understanding of underserved communities. As part of our Independent Practice Association (IPA), providers gain a partner that understands both the human and organizational aspects of delivering care.
We believe that every provider deserves a network that lifts barriers, enables better outcomes, and values your contribution—not just in metrics, but in the lives you impact.
Our IPA is designed to support and elevate the work of providers who care deeply about community health. Through our IPA, we offer:
Help navigating patient transitions, compliance needs, and care continuity.
Actionable reporting to identify gaps in care, reduce costs, and improve outcomes.
Proven experience in adapting to changing state and federal guidelines, including value-based care models.
A dedicated team that listens, understands, and acts with empathy and urgency.
Regular updates, learning sessions, and toolkits to help you serve patients more effectively.
We know what it takes to create practical solutions that work today and adapt for tomorrow.
Joining our IPA is straightforward and impactful. You don’t just gain access to a provider network; you gain a partner in care.
