2026 Provider Annual Attestation

Each year, Medicaid managed care providers are required to complete three brief attestation forms – DO, Employee, HIV forms. These forms are a NYS Medicaid Requirement. Below is a brief description of what each form will confirm.

To ensure Monroe Plan for Medical Care, (dba Elm and Oak Health) and Molina Health Plan of NY have the latest information on all affiliated providers, please complete the roster available at this link: Elm & Oak Health Roster Template and submit it to [email protected].

Access and Availability

Elm and Oak Health, together with Molina, upholds access to care standards and implements processes for ongoing monitoring of health care access, including behavioral health services, provided by contracted adult and pediatric primary care physicians (PCPs) and participating specialists, such as OB/Gyn, behavioral health providers, and both high-volume and high-impact specialists

Molina Provider Exclusion Monitoring Attestation

Molina Healthcare of New York, Inc. is required to confirm our network providers have procedures in place to identify and determine the exclusion status of your managing employees through routine checks of Federal databases.
